Fill in the form and submit it to ESIC for registration on the official website. 3. Once verified, a registration number, a 17-digit unique identity will be provided to the organization.
The first option is to visit your concerned ESIC regional branch office and submit them a request letter to issue a registration certificate, then they will allot a copy of your establishment's ESIC registration certificate.
To submit a re-imbursement claim bill/bills an IP has to write an application to the Administrative Medical Officer, ESI Scheme, Assam through the concerned Insurance Medical Officer i/c, ESI Scheme Dispensary along with the claim bill format which is available at Dispensary.
Is ESI contribution refundable? According to the provisions of Employees State Insurance Act, 1948, contributions towards Employees State Insurance Scheme are not refundable under any circumstance.
